I don't know, I mean...I'm sure he could've gone off to do something dark, but we really don't know that he left the hospital entirely. I mean, the last scene was clearly just supposed to be Dean and Cas. Sam could've just gone off to get coffee or some shit, or the writers just didn't want him in there for whatever reason. He was clearly worried about Dean throughout the rest of the episode, so I don't think he's just up and leaving him completely at a time like that. I could be wrong, but...I don't know, I'm not that worried about it yet.
